Upon finishing each level players have the opportunity to continue on playing or progress. The game time is organized by year and players cash and reputation are adjusted to reflect their performance. Players compete against computer rivals, named after characters such as Holly from Red Dwarf. Inspections by VIPs or health inspectors can see the hospital receive cash bonuses or fines, depending on its performance. Emergencies require a large number of patients to be cured within a short time. Epidemics see the rapid spread of disease and require all the carriers to be treated. Doctors can also be trained as specialists to allow the hospital to offer higher standards of treatment. Staff morale is important and rest areas must be provided. Nurses, doctors, and janitors, all must be hired.
